We looked for evidence that safety was a priority for everyone, and leaders embedded a culture of openness and collaboration. We checked that people were safe and protected from avoidable harm.

At our last assessment we rated this key question requires improvement. At this assessment the rating has remained requires improvement. This is because the assessment was focused on two specific areas and we did not review enough information to rerate this key question. The service breached regulations in safe and effective staffing and safe environments at our last assessment and these are the areas we reviewed at this assessment.

The service had a good learning culture and people could raise concerns. Managers investigated incidents thoroughly. People were protected and kept safe. Staff understood and managed risks. The facilities and equipment met the needs of people, were clean and well-maintained and any risks mitigated. There were enough staff with the right skills, qualifications and experience. Managers made sure staff received training and regular appraisals to maintain high-quality care. Staff managed medicines well and involved people in planning any changes.

The service detected and controlled potential risks in the care environment. Staff made sure equipment, facilities and technology supported the delivery of safe care.

Managers had reviewed the number of people who were trained as fire wardens in the department and the processes they followed to maintain safety in the event of a fire. Progress against an action plan showed there were enough staff who were trained as fire wardens to cover each shift. This meant the service was no longer in breach of legal regulations around safe environments. Managers continued to monitor the numbers of fire wardens in the department. Managers and staff had improved guidance for all staff. Action cards had been developed for staff to refer to. Fire wardens used tabards to clearly identify them in their role when on shift and if a fire occurred. Staff carried out checks of areas to ensure they were compliant with fire regulations and reported them to governance committees.

The service made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. Staff worked together well to provide safe care that met people's individual needs.

Staff and managers had reviewed staffing arrangements for the department and taken immediate actions to mitigate risks. Additional weekend shifts were offered to consultants to fill the shortfall and timings of shifts were adjusted to encourage greater choice. As a long term, more sustainable measure, two additional consultants had been recruited and were in post at the time of our assessment. This meant the service was no longer in breach of legal regulations around staffing. There were additional plans which had been approved by the executive team, to recruit 6 more consultants to the department which would provide more resilience to consultant staffing in the future. A plan was being monitored at governance meetings and completion was due for the Autumn 2025.
