We found there had been little improvement to the completion of care plans as well as the management and treatment of wounds.

Staff had not consistently developed care plans that met patients’ individual physical health needs.

At the last inspection we found patients with long term conditions, as well as those who required support with the management of a wound, did not have suitable care plans in place. We also found staff had not recorded sufficient information in their patient record regarding care and treatment provided.

During this inspection we saw little improvement in the completion of care plans for patients with long-term conditions. Care plans were generic and lacked relevant clinical information regarding the specifics of the patients’ condition which meant their condition may not be managed effectively.

We also found that whilst there had been improvement in the wound management and record keeping for some patients, other records lacked information. There was not always a care plan and people did not have their wound dressed regularly when this was required, this led to some patients developing an infection.
