Assessing needs

The provider did not always make sure people’s care and treatment were effective because they did not always check and discuss people’s health, care, wellbeing and communication needs with them. Some relatives felt that communication could be improved, particularly in relation to being kept informed about how their loved ones were doing on a day-to-day basis. Due to excessive information recorded in care plans, it was not always clear what was the most current information. Not all care plans and risk assessments evidenced that people had been fully involved in the creating and reviewing them. However, relatives spoken with confirmed they were generally involved in care plan reviews. One told us, “A copy of their care plan was sent to me and my views requested.” And “I am involved in the care plan.”

Overall, people’s holistic needs were suitably assessed. This included people’s communication support needs. For example, a ‘How to communicate with [service user]’ plan was in place to provide guidance for staff on what a person’s non-verbal communication suggested and how to respond.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Overall, the provider followed the principles of the Mental Capacity Act (2005). Where people lacked capacity to make decisions around their care and support, a relevant and specific mental capacity assessment was in place, and an associated best interest decision to record the decision made. We saw evidence of involvement of people, and those relevant to the decisions being made. Advice was sought from professionals to ensure people’s wishes could be safely respected, for example where a person lacked the capacity to understand the risks associated with smoking but wished to continue doing so.

Staff demonstrated an understanding of the MCA and how to support people to make decisions about their care. One staff member told us, “We will give people options so they can make their own choices.”
