Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.

Staff identified and supported carers and veterans, maintained registers and helped patients access relevant services and benefits. The practice also promoted national screening and vaccination programmes and took steps to encourage uptake among eligible patients.

Monitoring and improving outcomes

Score: 3

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The practice used audits and performance data to monitor and improve patient outcomes. Staff reviewed areas such as prescribing, cervical screening and childhood immunisations to identify opportunities for improvement. The service achieved 85% -90% uptake for childhood immunisations against a target of 95% and 74% uptake for cervical screening against a target of 80%. Whilst performance remained below national expectations, leaders had implemented recall systems and targeted follow-up of non-responders. Performance was regularly reviewed and discussed at clinical meetings and unverified data from 2026 demonstrated that the practice had achieved above the 80% target for cervical screening.

We did not look at Consent to care and treatment during this assessment. The score for this quality statement is based on the previous rating for Effective.
