The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity.
The provider worked to deliver continuity of care and support with regular staff allocated to people. People we spoke with confirmed this. Staff told us they understood the health and care needs of the people they supported.
