Staff responded to risks at the point of care and systems such as water safety and emergency preparedness were well managed. However, leaders did not always ensure risks were consistently identified, mitigated, and monitored. Environmental risks, medicines storage concerns and inconsistent storage of patient records meant people were not always protected from harm.
