Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. Peoples current care needs were reflected in their care plan. Care records showed appropriate health care professionals had been involved and needs were regularly reviewed.

Delivering evidence-based care and treatment

Score: 3

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. People and their relatives spoke positively about the care and treatment delivered by the staff team. One said, “They [staff] are responsive here, I need 24/7 care, and I feel it is well provided.”

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. The staff team worked well together and supported each other to ensure people's needs were met. One staff member said, “We work well as a team, we are consulted about everything.”

Supporting people to live healthier lives

Score: 3

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. Care plans evidenced people had access to health care professionals such as GPs and occupational therapists (OTs). Relatives told us staff reacted promptly to any changes in people’s needs and sought advice and guidance. One relative said, “There was an incident with [relative], but they were on the phone straight away seeking guidance, they explained to us what had happened, and they never try to cover anything up or hide things from us. They are very up front about everything."

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive and consistent and they met both clinical expectations and the expectations of people themselves. Care plans were reviewed and updated to reflect any changes to care and support. Daily notes and records were completed to monitor people’s health and wellbeing. People had access to local health and care services to support them to manage their wellbeing. Relatives told us staff looked at ways to continually improve outcomes. One said, “[Relative] has come on leaps and bounds since being here, especially in the last couple of months. The staff work really hard with them."

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. People’s care plans recorded their agreement to receive care and support and contained information from people about their wishes and choice about their care which was respected. One person said,” Staff respect all our choices. There is a choice each day for food, and I get enough brews and drinks through the day."
