Learning culture
At the last assessment we found not all incidents had been reported by staff and there was insufficient oversight of incident trends and missed opportunities for improvement.
During this assessment we found improvements in the reporting of incidents. Incidents were discussed at the daily safety meeting with managers checking that they had been reported into the ‘Datix’ incident reporting system. Records were kept of the daily meetings which provided an audit trail of each reported incident.
Incidents were allocated to an appropriate person to investigate with the Head of Healthcare and provider having oversight of the progress of each investigation. Any identified learning was shared with the wider healthcare team.
A new incident dashboard had been created which allowed managers to see at a glance the number of incidents reported, their progress and to monitor trends and themes.
