The practice had a governance system that included policies and procedures, which were accessible to staff and were reviewed on a regular basis.

There were processes for identifying and managing risks. We found staff were open to discussion and feedback, and staff worked together in such a way that where the assessment identified minor areas of risk which required improvement, these were acted on immediately.

Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records and complied with General Data Protection Regulations.

The practice had systems and processes for learning, quality assurance and continuous improvement. This included undertaking audits according to recognised guidance.

Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement.

Staff feedback was obtained through meetings and informal discussions. They were encouraged to offer suggestions for improvements to the service, and they said these were listened to and acted upon, where appropriate. For example, following staff feedback the practice implemented nurses swap surgeries and reception duties to equally share out opportunities and roles.

The practice gathered feedback from patients, the public and external partners, and responded accordingly. For example, following patient feedback the practice bought a second television for the bottom half of the waiting room.

The practice had taken steps to improve environmental sustainability. For example, the practice reduced the amount of single-use plastics used where possible, had sensor lights to reduce electricity usage, recycled where possible and had installed an electric vehicle car charger at the practice.

There were processes for identifying and managing risks. However, the provider should review these and make necessary improvements to ensure all recommendations from risk assessments were acted on in a timely manner and future staff recruitment was in line with Schedule 3 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

We noted improvements should be made to the systems for investigating incidents and accidents, and for receiving and acting on safety alerts. The practice should ensure external reporting of incidents and accidents is in line with relevant legislation.
