Nurse Plus is a domiciliary care agency providing personal care support to people in their own homes. We gave the provider 48 hours’ notice of our visit to ensure they were available to assist with our assessment. At the time of our assessment 68 people were receiving personal care and treatment. This service was previously rated as good. We carried out this assessment to confirm whether the rating of good remains accurate.
Prior to our assessment the service had experience some leadership changes. The provider had identified the impact of these changes on staff and people’s continuity of care. At the time of our assessment the provider had taken steps to strengthen quality monitoring and were working with people to ensure they received their care visits as planned.
People received safe, personalised care that was responsive to their needs. Where consistency of staff was achieved, positive and trusting relationships developed, supporting a good understanding of people’s preferences and enabling early identification of changes in need. Care plans were detailed, person-centred and regularly reviewed to ensure they remained relevant and effective. Risks were managed in a balanced way, promoting both safety and independence.
Systems supported safe medicines management, infection prevention and access to healthcare services. Staff communicated well with relatives and professionals, ensuring concerns were acted on promptly. They were supported through training, supervision and development, and described a positive culture where they could raise concerns and contribute to care planning.
Governance systems, including audits and spot checks, provided oversight of care delivery, and as leadership stabilised, the service showed improved structure, consistency and a continued focus on delivering high-quality care.
This assessment was conducted in line with the ‘Right support, right care, right culture’ guidance, which underpins how services for people with learning disabilities and autistic people should be delivered. We expect providers to uphold principles of dignity, equality, choice, independence, and inclusion. Nurse Plus met these principles consistently.
