CQC register Asana Lodge to carry out the following legally regulated services here:

Accommodation for persons who require treatment for substance misuse

  • Miss Carol Louise Talbott is responsible for these services.
  • Miss Jessica Elizabeth Sartain is the registered manager for these services at this location.
Condition of this registration relating to carrying out this regulated activity

The Registered Provider must within seven days, devise and implement an effective system to ensure that all past and future incidents occurring at Asana Lodge are reviewed, investigated and notified to the Care Quality Commission, and describe the learning from all incidents. The Registered Provider must provide:

a) confirmation that the system(s) implemented is effective, with the inclusion of the investigation report for each incident and lessons learned

b) details of action taken to ensure the system is being audited, monitored and continues to be followed

c) results of monitoring data and audits undertaken that provide assurance that action is taken to improve the quality and safety of services.

The Registered Provider must undertake and submit to the Care Quality Commission within seven days a comprehensive review of all clinical records for all service users, in particular but not limited to include:

a) care plans

b) risk assessments

c) crisis plans

d) unexpected exit from treatment plans.

The Registered Provider must undertake and submit to the Care Quality Commission within seven days a complete, immediate and continuing review of all ligature risk assessments and complete an action plan to mitigate all risks identified. The Registered Provider must ensure and provide evidence that staff have seen and understood the risk assessments.

The Registered Provider must undertake an immediate review and submit to the Care Quality Commission within seven days, of qualifications, competence, skills and experience of all staff at Asana Lodge in order to ensure that the needs of all service users are addressed. The review must include but not limited to:

a) List of all required training for the service type

b) name of staff member undertaking training

c) date of training,

d) nature of training,

e) name of training provider, and post-training competence checks.

The Registered Provider must submit to the Care Quality Commission within seven days, evidence of their application for a Home Office Stock license. The Registered Provider must, thereafter, submit to the Care Quality Commission by 5pm every Monday, evidence of compliance with safe medication management until the Registered Provider is in receipt of the license.

The Registered Provider must undertake a review of all internal governance systems and processes at Asana Lodge and devise appropriate policies within the next seven days, in particular but not limited to:

a) care record audits

b) risk assessment audits

c) investigations of poor staff performance

d) staff training compliance

e) medication management

f) lessons learnt

g) evidence of embedding actions through audit

h) supervision and appraisal audits.

The Registered Provider must provide the Care Quality Commission within seven days, with a report setting out the actions taken or to be taken in relation to the conditions one to seven. Thereafter, the Registered Provider must submit to the Care Quality Commission a fortnightly report detailing the progress made against the action plan by 5pm Monday of each fortnight commencing 14 days within receipt of this notice. The report must include:

a) details of the system(s) and processes that are implemented to comply with the conditions,

b) details and confirmation of action taken to ensure the system(s) are being audited and monitored to improve the quality and safety of services.

The Registered Provider must not admit or re-admit any more than 5 service users every two weeks, to a maximum of 10 service users at any time, without the prior written agreement of the Care Quality Commission
