Condition of this registration relating to carrying out this regulated activity

The provider must undertake a monthly audit of the following areas:
i. All service users' risk assessments that relate to assessing, mitigating and managing risks associated with their care;
ii. All service users' care plans to ensure they include person-centred detail around life histories, reflective of their needs and preferences, and end of life care choices;
iii. Staff attendance on care visits and the effectiveness of monitoring system for staff attendance
iv. All incidents, accidents, complaints and safeguarding concerns reported to the provider, any action taken to minimise risks to people and to implement lessons learnt from any trends and themes identified within the provider's review; and
v. How service users' consent for care was obtained, how their capacity was assessed and how decisions had been made in service users' best interests when required.
A summary report of the findings and actions in respect of each of the audits must be submitted to CQC on the first Monday of each month, with the first report to be provided by Monday 3 July 2023. The report must specify timescales for action to be taken and highlight who is responsible to ensure that the required improvements are made.
