About the service

Tanworth Court is a residential care home providing personal care to up to 60 people. The service provides support to older people and younger adults with physical disabilities, sensory impairment or dementia. At the time of our inspection there were 41 people living at the service.

People’s experience of using this service and what we found

Staff, people and their families worked together to assess and manage the risks people might face. People's support plans reflected their range of needs and this promoted their wellbeing and quality of life.

People were supported in a safe, clean and well-maintained environment that met their needs. People and their families or representatives were involved in discussions about how they received support. Staff supported people to make decisions in their best interests. People were supported to access specialist

health and social care support to maintain their independence, health and wellbeing.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

Staff evaluated the quality of support provided to people, involving the person, their families and other professionals as appropriate. The registered manager was keen to develop a culture of improvement and transparency. To achieve this, they completed regular checks on the care people received and identified actions to improve the quality of care and the environment.

Rating at last inspection and update

The last rating for this service was requires improvement (published 29 April 2022).

At our last inspection we found breaches of the regulations in relation to managing risks to people and a lack of oversight of the service. The provider was made aware of our concerns and the improvements which were required to be completed by 27 May 2022.

At this inspection we found improvements had been made and the provider was now meeting this regulation.

Why we inspected

We undertook this targeted inspection to check whether the Warning Notice we previously served in relation to Regulation 12 (Safe Care and Treatment) and Regulation 17 (Good Governance) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 had been met. The overall rating for the service has not changed following this targeted inspection and remains requires improvement.

We use targeted inspections to follow up on Warning Notices or to check specific concerns. They do not look at an entire key question, only the part of the key question we are specifically concerned about. Targeted inspections do not change the rating from the previous inspection. This is because they do not assess all areas of a key question.

During an inspection looking at part of the service

Tanworth Court is a residential care home providing personal and nursing care to up to 60 people. At the time of our inspection 49 people lived at the home and one person was in hospital. Some of those people lived with dementia.

People’s experience of using this service and what we found

Risks associated with people’s care were not always identified or responded to in a timely way. Quality assurance checks were not always effective, as they failed to identify the issues we found, such as the environment risks. Issues they did identify relating to poor record keeping, remained unresolved.

People felt safe living at Tanworth Court. Staff were recruited safely and felt supported by the management team.

A service improvement plan was under constant review and the provider took appropriate action to address the concerns we raised during the inspection. However, further time was needed to ensure improvements made were sustained.

Rating at last inspection

The last rating for this service was good (published 14 July 2021).

Why we inspected

We received concerns in relation to the safety of people living at the home. As a result, we undertook a focused inspection to review the key questions safe and well led only. For those key questions not inspected, we used the ratings awarded at the last inspection to calculate the overall rating. The overall rating for the service has changed from good to requires improvement based on the findings of this inspection.

We looked at infection prevention and control measures under the Safe key question. We look at this in all care home inspections even if no concerns or risks have been identified. This is to provide assurance that the service can respond to COVID-19 and other infection outbreaks effectively.

Enforcement

We are mindful of the impact of the COVID-19 pandemic on our regulatory function. This meant we took account of the exceptional circumstances arising as a result of the COVID-19 pandemic when considering what enforcement action was necessary and proportionate to keep people safe as a result of this inspection. We will continue to monitor the service and will take further action if needed.

We identified breaches in relation to people’s safety, the safety of the environment and governance of the service.

Follow up

We will meet with the provider following this report being published to discuss how they will make changes to ensure they improve their rating to at least good. We will work with the local authority to monitor progress. We will return to visit as per our re-inspection programme. If we receive any concerning information we may inspect sooner.

During a routine inspection

About the service

Tanworth Court is a residential care home providing personal and nursing care to 43 people aged 65 and over at the time of the inspection. The service can support up to 60 people.

People’s experience of using this service and what we found

People were safe in the home and were supported by staff who knew the risks associated with their care. Medicines were managed safely and people received them as expected. There were enough staff to meet people’s needs and keep the home clean.

People’s care needs had been assessed and staff had received training so they knew how best to provide the care. People enjoyed their meals and were supported to maintain a healthy weight. Where needed health care professionals had been contacted for direct care or support.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

People enjoyed living in Tanworth Court and liked the staff who supported them. People were treated kindly, promoted to be independent and had their privacy protected.

Care plans detailed people’s care needs and staff used these to provide the right care and support. People got to do the things they enjoyed and had contact with people who were important to them.

The management team were known to people and staff and the registered manager made regular check to make sure people were happy with their care.

This was a planned inspection to provide a rating for the service.
