About the service

La Vie en Rose is a domiciliary care service providing the regulated activity of personal care. At the time of the inspection, 31 people were receiving support from the service. Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do, we also consider any wider social care provided.

People’s experience of using this service and what we found

Improvements had been made since our last inspection to the provider’s staff recruitment processes and notifying CQC of any significant events.

We found safe recruitment systems were in place and being used to ensure staff were suitable recruited and vetted before supporting people.

Staff had a good awareness of the importance of recognising and reporting signs of abuse or any incidents or decline in people’s health. Managers reviewed any concerns and reported them to the relevant authorities and CQC.

Progress was being made to ensure people’s risk assessments were person centred and detailed to guide staff on people’s support requirements.

Staff understood their role to maintain high standards of hygiene, wear PPE and carry out regular Covid 19 testing in line with government guidance.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

We expect health and social care providers to guarantee autistic people and people with a learning disability the choices, dignity, independence and good access to local communities that most people take for granted. Right support, right care, right culture is the statutory guidance which supports CQC to make assessments and judgements about services providing support to people with a learning disability and/or autistic people. We considered this guidance as there were people using the service who have a learning disability and or who are autistic.

We have made a recommendation about staff training on how to interact appropriately with people who have a learning disability and autistic people, at a level appropriate to their role in line with current legislation. This will help to ensure staff have the right skills to support people with a learning disability and autism in the future.

There was no registered manager in post at the time of this inspection, however the nominated individual confirmed they would be applying to be the registered manager with CQC.

The service was led by managers who were approachable and were passionate about delivering person centred care. Managers focused on learning from incidents and identifying areas that required improvement with the aim to improve the management systems and people’s experiences of the service.

Effective systems were in place to manage and monitor the service and engage with staff and people who use the service.

Rating at last inspection and update

The last rating for this service was requires improvement (published 16 June 2021)

The provider completed an action plan after the last inspection to show what they would do and by when to improve. At this inspection we found improvements had been made and the provider was no longer in breach of regulations.

Why we inspected

We carried out an unannounced comprehensive inspection of this service on 13 May 2021. Two breaches of legal requirements were found. The provider completed an action plan after the last inspection to show what they would do and by when to improve in relation to ‘Fit and proper persons employed’ and ‘Notifications of other incidents’.

We received concerns in relation to people’s medicines. We undertook this focused inspection to follow up on the concerns raised with CQC and to check they had followed their action plan and to confirm they now met legal requirements. This report only covers our findings in relation to the Key Questions Safe and Well-led which contain those requirements.

For those key questions not inspected, we used the ratings awarded at the last inspection to calculate the overall rating. The overall rating for the service has changed from Requires Improvement to Good. This is based on the findings at this inspection.

We will continue to monitor information we receive about the service, which will help inform when we next inspect.

During a routine inspection

About the service

La Vie En Rose provides domiciliary care and support for people living in their own homes. At the time of our inspection there were 57 people who were receiving personal care. The service provided care for people with long term health care conditions, older people, people with physical disabilities, people living with dementia and people needing end of life care and support. The service was contracted by the local authority to provide complex care support. Care staff provide a service to people who need assistance with aspects of their care including mobility needs, personal hygiene and eating and drinking.

People’s experience of using this service and what we found

Safe recruitment practices had not always been followed and pre-employment checks had not always been undertaken to gather assurances about staff conduct.

The registered manager and provider had not always informed CQC of notifiable incidents.

People's individual risks were assessed and staff were given clear information on how to protect people from the risks associated with their care. Care staff had the training and experience they needed to meet people's needs.

People and their relatives received care and support from a consistent staffing team. Staff spoke positively about the time and support they received and how this promoted person centred care.

People were supported by staff, who were training to meet their individual needs. Staff spoke positively about the support they received from the registered manager and provider.

People and their relatives were informed of any changes and were involved in their loved ones care and support.

The provider and registered manager ensured lessons were learnt when incidents occurred or where concerns were reported. There were systems to ensure people's needs and risks were regularly reviewed and any healthcare concerns were identified and appropriate action taken.

Rating at last inspection

This is the second time we have inspected this service since it was registered in January 2020. The first time we inspected (7 October 2020) we undertook a targeted inspection and therefore did not rate the service.

Why we inspected

This was a planned inspection based on the service being newly registered.

We have identified breach in relation to regulation 19 (Fit and proper persons employed) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 at this inspection. We also identified a breach in regulation 18 (Notification of other incidents) of Care Quality Commission (Registration) Regulations 2009.

Follow up

We will meet with the provider following this report being published to discuss how they will make changes to ensure they improve their rating to at least good. We will work with the local authority to monitor progress. We will return to visit as per our re-inspection programme. If we receive any concerning information we may inspect sooner.

Rating at last inspection

This is the first time we have inspected this service. As this is a targeted inspection the service remains unrated.

Why we inspected

We undertook this targeted inspection to follow up on concerns we had received about people’s care and how the service managed people’s individual risks. A decision was made for us to inspect and examine those risks.

CQC have introduced targeted inspections to follow up on Warning Notices or to check specific concerns. They do not look at an entire key question, only the part of the key question we are specifically concerned about. Targeted inspections do not change the rating from the previous inspection. This is because they do not assess all areas of a key question.

We will work alongside the provider to monitor the services overall progress. We will return to visit as per our re-inspection programme. If we receive any concerning information we may inspect sooner.
