Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. Records showed that people received an assessment of their needs prior to their care and support commencing. The service regularly reviewed people’s care and support to ensure that the care provided remained reflective of people’s needs.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. People’s care plans contained information about how to support people in line with the advice of health and social care professionals.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Professionals told us the service worked well with them. A professional told us, “The services responds quickly to queries, you can tell they [staff] know the people they support well.” Staff told us they worked well together and had regular team meetings where they could share feedback about their work.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. People had access to healthcare professionals and staff adhered to their advice. People’s care plans included information to enable to staff to identify issues at the earliest opportunity, including how people would like to be supported should there be a decline in their health.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. The provider kept daily records of interventions and took appropriate steps to escalate concerns as required.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. The provider ensured that people had consented to their care and treatment. Where people required support to make decisions in relation to their care and treatment, the provider followed the principles set out in the Mental Capacity Act 2005 and recorded details of any lasting powers of attorney that were in place.
