• The service had effective systems to identify and manage risks to patient safety. The consultant psychiatrist worked closely with other agencies to assess and manage risks and understood safeguarding protocols and procedures.
• The service conducted comprehensive assessments of patient needs in a timely and efficient manner. Assessments included details of the patient’s mental health, social circumstances and family dynamics. Care records were individualised for each patient and had a focus on recovery.

• Patients and families were complimentary about the manner the consultant informed them of treatment options and involved them in care planning.
• The service worked in partnership with other experienced independent practitioners to collaborate, cross-refer patients and discuss clinical matters. There were governance systems in place that ensured the quality and efficacy of the service provided.

However:

• The service did not have robust alarm arrangements for lone working.
