Well-led
We found this practice was not providing well-led care in accordance with the relevant regulations.
The provider had made insufficient improvements to put right the shortfalls we found at our inspection on 16 January 2026. We have told the provider to take action. We will be following up on our concerns to ensure the provider has made the required improvements.

Governance, management and sustainability
At the inspection on 16 April 2026, we found the practice had made the following improvements to comply with the regulations:
Systems for ensuring policies and procedures were kept up to date had been improved and a compliance package was in use to support the effectiveness of the systems and ensure policies and procedures are reviewed on a regular basis. However, there was an ongoing lack of oversight and leadership at the practice which led to continuing omissions and ineffective systems.
The practice had ineffective systems and processes for learning, quality assurance and continuous improvement. Although audits were undertaken, these were not completed according to recognised guidance. Action is needed to ensure audits of radiography, infection prevention and control, record keeping and antimicrobial prescribing are undertaken at regular intervals to maintain or improve the quality of the service. The practice should also ensure that, where appropriate, audits have documented learning points, and the resulting improvements can be demonstrated.
We saw that the practice had implemented systems for identifying and investigating incidents and accidents. However, improvements were required to the processes for managing risks.
Improvements had been made to the complaint handling procedures, including establishing an accessible system for identifying, receiving, recording, handling and responding to complaints by service users.
