Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question inadequate. At this assessment the rating has changed to good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. For example, we saw incidents and accidents, safeguarding concerns and complaints were recorded on the provider’s system. These were reviewed, discussed and analysed to determine any underlying causes or trends and to identify any actions needed to ensure the safety of the people living at Naylorsfield and Hartsbourne. The Registered Manager, Learning Disability Nurse and Team Leaders were proactive in seeking feedback and opportunities to learn and improve the service.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. People and families told us they were confident staff would act when additional advice and support was needed. One persons loved one said, “Been a massive culture change and [registered manager] has driven that, and [registered manager] listens to your concerns and put things in place. They include family and they all listen to [Name], and the communication is key, and you need that level of communication in this type of home.”

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. One health professional shared with us, “The [Registered manager] attended all relevant meetings and provided reassurance around managing risk of both patients, they offered a holistic wrap around service to ensure individuals being discharged from hospital are discharged into a safe, secure, and caring environment.” Staff were trained in safeguarding. Staff we spoke with described with clarity what a safeguarding concern was, and they would not hesitate to report any concerns they had and were confident this would be acted on.

Involving people to manage risks

The provider worked with people to understand and manage risks. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. People said staff knew their needs and met these safely. There were effective processes in place to ensure risks to people were assessed and mitigated. Risk assessments were clear, comprehensive and regularly reviewed. A member of staff told us, “We drive improvement and what matters to the people who live here. We are passionate about helping the people here achieve what they want for their future in a safe and caring environment” and another said, “Since I joined I feel the people we care for can depend on us as staff to help them find solutions for what they want from their lives without judgement but in a proactive and constructive way.”

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. People were supported in a safe and well-maintained environment that met their needs. People and relatives told us they liked the environment they lived in. The home had been developed to meet the needs of people living at Naylorsfield and Hartsbourne with people at the heart of the decisions to make it a safe and homely environment. There were effective systems in place to monitor and regularly check the safety and upkeep of the premises. The management team and staff worked together to help ensure any potential risks were identified and addressed promptly such as faulty equipment or trip hazards. Safety checks were completed daily and weekly in all areas of the home to ensure safe systems were in place. Naylorsfield and Hartsbourne was spacious and modern with many comfortable areas for sitting and chatting. The lounges were well set out and comfortable and we found people could personalise their bedrooms as they wished.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. Staff recruitment processes were in place with relevant checks of staff’s employment history, character, right to work, and checks with the disclosure and barring service (DBS). Staff we spoke with were very committed and passionate about the work they did at Naylorsfield and Hartsbourne. Staff told us, “Morale is super excellent” and “We come here every day to try and support the people and put a smile on their faces and if we can’t then we support them best we can. We are a team that doesn’t sit back, we are always looking to improve, and we have the best team since I have been here.” One person’s loved one told us, “The staff [Name], [Names] and [Names] supported [Relative] a lot and they have made themselves available and take [Relative] on walks. Taken good care of him mentally and physically especially since we had a family bereavement.”

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Shared areas of the home as well as people’s own rooms were clean and tidy. The home was clutter free. Staff wore personal protective equipment (PPE) appropriate to their tasks. People were supported to maintain good hygiene. Staff knew about infection prevention procedures which they followed to protect people from risk.

Medicines optimisation

The provider made sure medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. People received their medicines safely and as prescribed. People’s care plans were detailed and reflected their preferences about how they wanted to take their medicines, and this was respected. Medicines were stored appropriately in a locked cabinet and kept in a cool room. Temperatures were taken and recorded regularly and were within safe range. The registered manager undertook regular audits to ensure people were receiving their medicines as prescribed. Where errors were identified, we saw appropriate actions were taken.
