Governance, management and sustainability

Score: 3

The service had clear responsibilities, roles, systems of accountability and good governance.

Leaders and managers supported staff, and all staff we spoke with were clear on their individual roles and responsibilities. We saw evidence that their regular appraisals and performance reviews for staff working at the service. The provider had established governance processes that were appropriate for their service. Managers held regular practice meetings, during which they discussed clinical concerns and emerging risks. Minutes and actions were clearly recorded any actions and shared with all staff. Staff took patient confidentiality and information security seriously.

At the last inspection, we found that consent forms and patient information regarding potential complications were not consistently completed for the vasectomy service. During this inspection, we found that improvements had been made. Consent forms are now fully completed and uploaded to patient records, and an internal system check ensures that a record cannot be saved without the associated consent form. This process applies to minor surgeries and vasectomy procedures.

The practice had also conducted audits since the last inspection, including audits of vein surgery completed within the last year to ensure compliance and patient safety.
