During an assessment under our new approach
Date of assessment: 18 July 2025 to 8 August 2025.
Threeways Dom Care is a supported living service providing personal care for up to 3 adults with a learning disability. At the time of our assessment, 2 people were using the service.
The service registered with the Care Quality Commission (CQC) in November 2018. This is their first assessment and rating since registering.
The provider was in breach of legal regulations relating to person-centred care, safe care and treatment, mental capacity, safeguarding, good governance and staffing.
We assessed the service against ‘Right support, right care, right culture’ guidance to make judgements about whether the provider guaranteed people with a learning disability and autistic people respect, equality, dignity, choices, independence and good access to local communities that most people take for granted. We identified the provider was not meeting the standards of care set out in this guidance.
People were at risk of financial and material abuse. Any accidents or incidents that had occurred were not recorded appropriately to investigate triggers and inform if any lessons needed to be learnt. There were not enough staff to meet people’s needs safely, and staff’s recruitment files did not always confirm that they had been recruited safely. Medicines management and mitigating risks to people needed to improve. People’s rights were being restricted unlawfully and staff’s knowledge of The Mental Capacity Act and consent was poor. We identified areas where documents had been falsified and where the management team had not been open and transparent with us.
However, most staff were complimentary of the management team and said they were approachable and supportive.
In instances where CQC has begun a process of regulatory action, we may publish this information on our website after any representations and/or appeals have been concluded, if the action has been taken forward.
This service is being placed in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we use our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.
