At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff investigated and reported concerns about safety appropriately. Lessons were learnt to continually identify and embed good practice. Processes evidenced an oversight of incidents, accidents and complaints. The registered manager could evidence how lessons were learnt and shared with staff. Feedback collected from staff was used to continually make improvements and embed good practice. Staff told us they were confident to raise concerns, and felt the management would take action as required. All feedback was shared with staff, positive or negative.

 

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. The registered manager worked with people and their relatives to develop an assessment of the person’s needs prior to a care package commencing. The provider worked with external professionals to ensure seamless transitions into care.

 

 

 

 

 

 

 

 

 

 

Safeguarding

Score: 3

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. Managers and staff understood how to safeguard people. Staff were up to date with safeguarding training and could demonstrate they understood how to raise safeguarding concerns.

Involving people to manage risks

Score: 3

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. Risk assessments were person centred, and the registered manager had implemented systems to ensure that risk assessments were updated with people’s involvement.

Care plans were person centred, and risks and mitigations were clearly identified in the assessments. Care plans and risk assessments were completed with people and their families and we saw evidence of their involvement and consent. We reviewed daily notes and observations made by staff after each care call and care was delivered in line with care plans and people were happy with the care they received.

The registered manager told us they worked with people and their families to involve people in managing their risk. They told us, “eople we support attend their meetings, we work with families and involve professionals where needed’.

 

 

Safe environments

Score: 3

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

The provider conducted regular environmental risk assessments to ensure the environment, equipment, facilities and technology supported the delivery of safe care. Staff received mandatory moving and handling training to ensure they used equipment safely. People’s equipment needs were detailed in their care plans and risk assessments were completed to ensure safe use and practice.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. Processes in place ensured staff had been recruited with a range of checks including references and disclosure and barring checks (DBS). Staff had been trained appropriately and were up to date with training. Staff received regular supervisions and appraisals .Feedback from people and their relatives was positive, comments included, “[Staff] arrive on time, yes they have the skills to care for me, they are really good.” and “I have one care worker and they are very good, [care worker] knows my needs and supports me with everything, makes my breakfast and asks what I want to eat [care worker] never refuses to do anything for me.”

 

 

Infection prevention and control

Score: 3

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. The provider had a infection control policy in place and carried out infection control audits. Staff received infection prevention and control training and had access to appropriate personal protective equipment [PPE].

 

 

Medicines optimisation

Score: 3

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. The provider had a medication management policy in place. People received their medicines as prescribed, by staff who were trained in how to administer medicines safely. Staff competency to administer medicines was checked and reviewed by the registered manager.
