At our last assessment we rated this key question Requires Improvement. At this assessment the rating has remained. This meant the management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care.

The service was in breach of legal regulation in relation to good governance.

Shared direction and culture

Score: 3

The provider had a shared vision and values. Staff and leaders demonstrated wanting to promote a positive, compassionate culture.

Since our last inspection, the provider had worked to improve the culture in the service. We saw this was discussed as a regular topic in staff meetings and supervisions. Although we saw improvements had been made, work on areas such as use of respectful language in written records was ongoing and still being embedded.

People appeared relaxed and happy around staff, and we saw positive interactions between people and staff. Staff spoke about people with kindness, and passionately about their roles.

Capable, compassionate and inclusive leaders

Leaders did not always have the skills and knowledge to lead effectively.

Although leaders were responsive when we identified and shared concerns, we could not be assured concerns would be identified independently. We found, and a provider audit also identified, shortfalls in management’s knowledge and understanding of the Mental Capacity Act 2005.

All staff we spoke with shared positive feedback about the registered manager. A staff member said, “They are approachable, friendly and have good communication skills”, and another said, “They are sensitive, responsive and caring”. A health and social care professional described how the registered manager had taken positive action to improve communication with a person’s relative in response to feedback.

Freedom to speak up

The provider fostered a positive culture where people felt they could speak up and their voice would be heard.

All staff, people and relatives we spoke with shared positive feedback about management of the service and felt concerns would be acted on. A staff member told us about a medicines error they had made and what actions were taken to reduce the risk of further incidents. This showed they felt both able and safe to speak up when they realised their error, and safe to share this with external organisations.

Workforce equality, diversity and inclusion

The provider valued diversity in their workforce. They worked towards an inclusive and fair culture by improving equality and equity for people who worked for them.

Staff comments included, “It’s quite a diverse team so we have to be aware of our cultural differences. As a team we have to account for those things, we’re all bound by the end goal of providing a good service”, and “The team work well together and support each other”.

Governance, management and sustainability

The provider did not operate robust systems and processes to effectively assess, monitor and improve quality and safety. Although we found some improvements since our last inspection, timely action had not been taken to address all breaches and shortfalls. A provider audit completed in May 2025 noted audits were not always identifying areas of concern, stating they were ‘tick box with no oversight or review’. Although the provider identified this in May 2025, we found audits were still ineffective at identifying shortfalls and action plans did not show any increase in provider oversight. This exposed people to a risk of ongoing harm.

Partnerships and communities

The provider understood their duty to collaborate and work in partnership. They shared information with partners and collaborated for improvement.

We received positive feedback from health and social care professionals about how the service worked with them. Comments included, “Bonhomie always update me with any information”, and “Yes information provided by service is relevant to client’s needs”.

Learning, improvement and innovation

The provider did not always focus on continuous learning, innovation and improvement. A provider audit completed in May 2025 found it was difficult to establish if lessons were learnt following incidents, there was a delay in updating care plans and staff were left working without correct guidance and documentation. We found continued shortfalls in these areas during our inspection. This meant effective action was not taken to learn and improve.
