Risks were assessed and mitigated. Staff understood people’s individual risks and protected them from harm. Staff were recruited safely and completed all relevant training. Lessons were learnt and shared with staff.

Learning culture

The provider and registered manager had systems in place to ensure lessons were learnt and shared to prevent reoccurrence. Incidents were reviewed and trends and patterns identified.

Staff told us the registered manager shared learning with them. One staff member said “Yes, they (registered managers) do share with us everyday, regularly through the group chat and personally whenever required.” The registered managers understood their responsibility under the duty of candour. The duty of candour requires providers to be open and honest with people when things go wrong with their care, giving people support and truthful information.

People told us staff learnt from their mistakes. People told us they could raise concerns with staff or the registered managers.

Involving people to manage risks

People told us they felt safe with staff. One person said, “I feel safe and have no real problems.” A relative told us, “I am happy that [person] is safe when they (staff) are here. They get [person] washed and dressed, put their creams on and make sure [person] takes the pills."

Staff told us they understood people’s risks and this information was documented on their systems. One staff member told us, “All of the risk assessments are available in the app to read anytime. Every carer is involved in updating them since we are the staff who feedback the updates.”

People had risk assessments in place for environmental risk as well as individual risks. We found a few hazards that had not been fully assessed. However, these did not put people at risk of harm and the registered manager agreed to implement these immediately.

Safe and effective staffing

Staff were recruited safely. The provider completed pre-employment checks such as references and Disclosure and Barring Service (DBS) checks. The Disclosure and Barring Service carry out a criminal record and barring check on individuals who intend to work with children and vulnerable adults, to help employers make safer recruitment decisions.

Staff told us they completed an induction and shadowed shifts before lone working with people. Staff were happy with their training and felt supported by their manager.

People told us they did not always know who was coming to support them. One person said, “I don’t get a rota so it's pot luck who's coming. However, I have found them all to be polite and friendly, I have become good friends with a lot of the carers.”
