Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

UK Health Security Agency (UKHSA) data for the period of 1 April 2024 to 31 March 2025 showed the practice had met the World Health Organisation (WHO) minimum recommendations for the uptake of 1 of 5 indicators relating to childhood immunisations. The other 4 indicators were above 80%.

NHS England data from June 2024 showed the practice had not met the target for uptake of cervical cancer screening for eligible patients aged 25 to 49 years (70.5%) or for eligible patients aged 50 to 64 years (78.6%). The target for both age groups was 80%. However, the scores were in line with other practices in the local area, and the service had clear proactive recall systems for both immunisation and screening, which included patient education events, appointments with a practice nurse and information which patients could take away and read.
