At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People had detailed assessments covering a range of areas such as health and communication. These were used to develop personalised care plans, which included information about people’s preferences and goals.

Delivering evidence-based care and treatment

Score: 3

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. People were supported to have enough to eat and drink which met their preferences. Staff encouraged people to participate in meal preparation to help develop their skills.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Where required, referrals were made to health professionals for additional advice and guidance, including therapy services. Their recommendations were included in people’s care plans to help ensure people were supported appropriately with all their needs.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. Staff supported people to make positive choices about their health and wellbeing, such as encouraging physical activity.

Monitoring and improving outcomes

The provider monitored all people’s care to continuously improve it. They ensured outcomes were positive and consistent, and that they fully met the expectations of people themselves. Staff supported people to achieve positive outcomes and aspirations, such as new experiences and developing new skills. This included fulfilling lifelong ambitions and new technology to enable people to become more independent. Regular reviews were carried out to help track progress against goals.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Mental Capacity Act (MCA) assessments and best-interest decisions were carried out where people lacked capacity to make decisions. Care plans included guidance on how to support people with choices and decision-making. Staff had completed training on the MCA.
