Staff identified and managed environmental risks effectively. Facilities and equipment were clean, well maintained, and suitable for service delivery, with appropriate risk assessments and maintenance contracts in place. A business continuity plan was established and regularly reviewed to ensure service resilience during disruptions. All staff had completed health and safety and fire training.

The leaders had sought advice on ensuring the waiting room met the needs of people living with dementia.

Safe and effective staffing

The service employed a range of clinical and non-clinical roles, which included GPs, nurses, and pharmacists. Leaders ensured staff were up to date with their training which the service had deemed mandatory and operated within their agreed areas of competence. The service followed safe recruitment procedures when employing staff, which was in line with national legislation. This included identity checks, review of qualifications, obtaining of professional references, immunisation status and a criminal records check.

The leaders made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision, development and appraisals. They worked together well to provide safe care that met people’s individual needs.

The leaders had recently commenced audits of clinical staff patient consultation records to ensure clinical competence.

Infection prevention and control

The service assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

The staff effectively assessed and managed infection risks. They promptly identified and controlled potential spread, sharing concerns with relevant agencies. During the onsite visit, the practice premises and sampled equipment were observed to be visibly clean. A designated infection prevention and control lead was in place, and all staff had received appropriate training. Cleaning schedules were followed, and audits and risk assessments were completed annually with actions taken to reduce risks. All staff had completed infection prevention and control training.

The leaders had carried out a hand hygiene audit in September 2025 where they had met 100% compliance.
