Supporting people to live healthier lives

Score: 3

The practice supported people to manage their health and wellbeing, promoting independence, choice and control while helping to reduce future care and support needs. For example, the social prescriber signposted patients to food banks, community fridges and cost-of-living support services.

Staff promoted healthy lifestyles and preventative healthcare by facilitating access to local wellbeing initiatives, including GP exercise referral schemes, community weight management programmes, and free swimming and badminton memberships for people over 50. The practice also organised weekly walking groups to reduce social isolation and improve physical health.

Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including smoking cessation and tackling obesity.

Monitoring and improving outcomes

The practice routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people using the service. They had a rolling programme of audits to assess compliance with monitoring and safe prescribing guidelines, for example, audits relating to new cancer diagnoses, prescription security, the two-week wait pathway and prescribing practices.

The practice provided unverified data to demonstrate they had achieved a 77% childhood immunisation uptake for all areas from 1 April 2025 to 30 March 2026. This is below the World Health Organisation target of 95% and below their achievement for 2024 to 2025. In response the lead nurse was working with the community to improve uptake through community engagement, outreach clinics, education and flexible appointments.

The practice provided unverified data to demonstrate they had achieved the national targets for the uptake of cervical screening from 1 April 2025 to 30 March 2026

Effective systems were in place to support the timely review of test results and oversight of referrals such as urgent suspected cancer referrals made through the two-week wait pathway. Clinical performance was routinely reviewed to evaluate and improve the quality of care. The practice regularly reviewed significant events and other clinical issues to support learning and improvement.
