We found this practice was not providing well-led care in accordance with the relevant regulations. We will be following up on our concerns to ensure they have been put right by the provider.

During our assessment of this key question, we found a lack of systems or processes that enabled the registered person to assess, monitor and improve the quality and safety of the services being provided. There was also a lack of systems or processes to assess, monitor and mitigate the risks relating to the health, safety and welfare of service users and others who may be at risk.

This resulted in a breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

The practice had policies and procedures in relation to the provision of conscious sedation. However there was a lack of governance and oversight for monitoring these procedures:

There was a lack of monitoring to ensure that the practice policies were in accordance with relevant guidelines, specifically in relation to the use of multiple sedative medicines.

There was a lack of oversight for monitoring the practice policies and ensuring that all staff were adhering to the practice procedures.

The practice did not have systems to audit the provision of sedation and dental records to ensure that they were clear, accurate and legible.

Following our inspection visit the provider submitted evidence of the changes they had and were implementing to improve the monitoring the provision of conscious sedation. We will carry out a further visit to check on these improvements.
