Our view of the service
We carried out this announced on-site focused assessment on 27 November 2024. This was carried out to review the arrangements for the provision of conscious sedation and to assess whether the practice was meeting legal requirements.
We found the practice had not met all regulations.
The practice did not have effective systems to assess and manage risks in relation to conscious sedation.
The practice did not have effective systems to review and investigate incidents in relation to conscious sedation.
The practice did not have a culture of effective leadership to monitor and improve procedures in relation to conscious sedation.
Following our inspection the provider stopped providing sedation services while they reviewed their procedures and submitted details of the improvements made to ensure the safety of patients. We will re-visit to follow up on these improvements.
Perfect Smile Reading Sedation Centre is part of Perfect Smile, a dental group provider. The practice is in Reading, Berkshire and provides NHS dental care and treatment for adults and children. The practice provided advanced sedation to adults and children.
There was step-free access to the practice and car parking spaces, including dedicated parking for disabled people, were available near the practice.
The practice had 3 treatment rooms, During the assessment we spoke with 1 dental nurse, the practice manager and the registered manager.
The provider was not complying with regulations 12 and 17.

We have asked the provider for an action plan in response to the concerns found at this assessment.
