Person-centred Care

Score: 4

The provider was exceptional at making sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in their needs.

Care plans were detailed, person-centred and reflected a thorough understanding of the people being supported. Staff recognised that achieving positive outcomes sometimes required creative and innovative approaches, and they consistently sought solutions that enabled people to live as independently, safely and freely as possible. For example, a person who preferred his own space were moved to an upper floor room which enabled them to have a bedroom and lounge area. The provider told us how this had significantly reduced the person displaying agitation and they had interacted more with staff in their familiar environment. Although the person had limited furniture in their bedroom due to safety reasons, the provider had made adjustments to ensure the person had easy access to their personal items. Such as, rails and shelves had been installed instead of a wardrobe.

The provider made meaningful and individualised reasonable adjustments to support people's wellbeing and quality of life. These adjustments were not focused solely on managing risk, but on enabling people to continue living in ways that reflected their preferences and needs. For example, one person was supported to live more comfortably and independently by being accommodated on an upper floor where they had access to both a personalised bedroom and dedicated living space. As the individual chose to spend significant amounts of time within this area, adaptations had been made to ensure the environment remained safe whilst preserving their freedom and comfort.

The provider demonstrated an outstanding understanding of individual sensory needs and the importance of responding positively to behaviours of distress. For one person who was known to engage in self-injurious behaviours to meet sensory needs, staff worked proactively to identify specialist equipment and environmental adaptations that promoted both safety and wellbeing. This included supporting the person to access suitable protective headwear and installing padded wall surfaces, reducing the risk of injury while ensuring the person's sensory needs continued to be met in a dignified and person-centred way.
