Safe environments

Score: 3

The service detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

Both the main site and branch site buildings were owned by the provider. There was a cleaner who worked across both sites. The provider worked to ensure both buildings were maintained. Staff and leaders completed health and safety risk assessments and undertook audits to ensure they had identified and addressed all risks. However, at the time of our assessment, cleaning supplies, including COSSH substances, were stored in a cupboard that was unlocked. Practice leaders told us that the cupboard should be locked, and we saw there was a working lock in place. There were appropriate risk assessments in place. Following our assessment, practice leaders raised this concern with cleaning staff. Leaders told us the cupboard being unlocked was human error on that day and staff had been reminded this should be locked going forwards. The service had a business continuity plan which was regularly reviewed and outlined how the service should continue to operate in the event of a disruption. Practice leaders were able to provide an example of a disruption to systems out of their control that occurred, where staff used the business continuity plan to good effect.

Safe and effective staffing

Score: 2

The service did not always follow safe recruitment procedures when employing staff, which were in line with national legislation. Specifically, the service did not always make sure there were records of staff immunisations available. In addition, we saw that the recruitment policy had not been reviewed in line with the review date. Following our assessment, we saw evidence that practice leaders had since collected this information from staff and had updated the practice’s recruitment policy to include immunisation status as a pre-employment check. Staff that had been employed since the fundamental standards were introduced in 2014 did have identity checks, review of qualifications, obtaining of professional references and a criminal records check. The service did make sure staff received effective support, supervision and development.

The service employed a range of clinical and non-clinical roles. Leaders ensured staff were up to date with their training which the service had deemed mandatory and operated within their agreed areas of competence

Infection prevention and control

Score: 3

The service assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

The service had cleaning schedules available, which outlined how staff should clean the building and its equipment. The service demonstrated how these were monitored for completion to maintain oversight of cleaning arrangements. During our onsite visit, the service's premises and a sample of equipment reviewed was noted to be visibly clean. The service’s infection prevention and control lead conducted regular risk assessments and audits to ensure compliance and took action where necessary to mitigate any identified risks. Staff had completed relevant training in infection prevention and control.

Medicines optimisation
