Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focused on identifying risks to people’s health, including ensuring housebound people with diabetes received increased monitoring and did not miss their annual review. Staff supported national priorities and initiatives to improve population health, including smoking cessation and tackling obesity. For example, people who smoked were sent text message reminders about available support and could self-refer to a local smoking cessation service.

The main site had a ‘pod’ where people could independently record their blood pressure, pulse and weight. People could enter their details directly into the system and their electronic record would be updated with the most recent readings. This enabled people to undertake these checks without an appointment and support self-monitoring of their health.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service met national targets for the uptake of childhood immunisations.

The most recent verified cervical screening data available at the time of this assessment was from June 2024. This showed the service had not achieved the expected minimum uptake rate of 80%, with screening rates of 71% for people aged 25 to 49 years and 73% among those aged 50 to 64 years. However, unverified data collected by the service in 2025 suggested the 80% minimum uptake target had been achieved for both age groups.

The service performed slightly below the national average for several cancer screening programmes, including breast and bowel screening. For example, breast screening coverage for people aged 53 to 70 years old was 67% in 2024/25, compared to the national coverage of 70%. The service’s website provided health advice and information on topics including breast screening to raise awareness and increase screening.
