Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff proactively identified risks to people’s health, including those approaching the end of life and those with caring responsibilities. They supported national priorities and initiatives aimed at improving population health, such as smoking cessation and tackling obesity.
The practice offered a range of health assessments and reviews, including NHS health checks and annual health checks for people with a learning disability, to help monitor and improve outcomes for patients.
Staff had received training in care navigation skills, enabling them to effectively signpost patients to the most appropriate services, such as community pharmacies for minor ailments and the social prescribing team for wider non-clinical support.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service actively promoted attendance for long-term condition reviews to support the ongoing monitoring and improvement of people’s health outcomes. Systems were in place to identify when reviews were due. Reminders were included on prescription slips for patients receiving medication, while those not prescribed medicines were contacted via text messages or letters.
The service met national targets for the uptake of childhood immunisations, with all indicators exceeding the national target of 90%. Data from 2024 showed that 83.1% of women eligible for cervical screening had received an adequate test within the previous 5.5 years, which was above the national target of 80%. The practice actively followed up people who did not attend screening appointments to encourage uptake.
The service had also worked collaboratively with the Primary Care Network to improve uptake following a reduction in cervical screening attendance, demonstrating a proactive approach to improving population health outcomes.
