Equity in experiences and outcomes

Score: 3

Staff and leaders actively listened to information about people who were most likely to experience inequality in experience or outcomes. They tailored their care, support and treatment in response to this.

Staff treated people equally and without discrimination. Leaders proactively sought ways to address any barriers to improving people’s experience and worked with local services. The practice was a veteran friendly practice and ensured that veterans were appropriately identified and coded on the clinical records to support personalised care and access to relevant services.

Staff understood the importance of providing an inclusive approach to care and made adjustments to support equity in people’s experience and outcomes. Staff told us reasonable adjustments were routinely made for patients requiring longer appointments, accessible information and learning disability support. For example, the practice worked with expectant and new mothers, particularly those from harder to reach communities, to promote breastfeeding and improve maternal and infant health outcomes.

Staff followed processes to identify and support patients who may face additional barriers in accessing services. This included recognising vulnerable patients such as those experiencing literacy challenges and ensuring they were appropriately coded to enable reasonable adjustments and tailored support.

The practice had access to a mental health practitioner through the Primary care network (PCN). This supported patients with their mental health needs and improved access to appropriate specialist support within primary care.
