Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health. Including a focus on lifestyle medicine and healthy living.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service had met national targets for the uptake of childhood immunisations for aged 2, however, was just beneath for 5-year-olds who had received Measles, mumps and rubella (MMR) 2 doses. Achieving 88%, (target 90%). The surgery followed up with patients who did not bring their children in for these vaccines (data from April 2024 to March 2025).

The service met national targets for cervical screening for the age band 50-64 years old, however, had achieved beneath target with 75.8% for people aged 25-49 years old being screened where the target is 80% (Data to June 2024). The surgery had taken steps to tackle this by offering before work appointments from 7.30am and evening appointments to capture those who had not attended. They sent regular reminders to those who did not attend.
