During an inspection looking at part of the service

We inspected Leegomery Surgery on 4 September 2013, 24 June 2014 and 18 September 2014 and saw that the practice did not meet required standards in supporting staff. At our last inspection we saw that suitable arrangements were still not in place to ensure senior staff received the appropriate training, professional development, supervisions or appraisal they needed to support their staff.

We told the practice to take immediate action and shared our findings with local stakeholders. We returned to the practice on the 3 December 2014 to check that the required improvements had been made and saw that they had.

During an inspection looking at part of the service

We inspected Leegomery Surgery on 4 September 2013 and found that the practice did not meet required standards in infection control and the recruitment and supporting of staff. We returned to the practice on the 24 June 2014 to check that the required improvements had been made. During that inspection we found that staff and patients were not protected from exposure to health care associated infections or unsafe or unsuitable equipment. We also found that the provider did not have effective recruitment procedures in place or suitable arrangements that ensured staff received appropriate training and support. We asked the provider to send us an action plan informing us what they would do to address these issues. The provider said they would address these issues by 15 September 2014.

At this inspection we found that staff had not received the appropriate immunisations to protect them against the identifiable risks of health care associated infections. However, appropriate action had been taken to protect patients from the risks of unsafe equipment. There had been improvements in their recruitment systems. Systems had been put in place to ensure that administrative and nursing staff received appropriate training and support. However, we found that the registered manager and practice manager had not received appropriate training or support.

During an inspection looking at part of the service

At our previous inspection on 4 September 2013, we found that the provider had not taken reasonable steps to keep patients safe from the risks of the spread of infection. At this inspection we found that patients were cared for in a clean, hygienic environment but staff were not protected from exposure to health care associated infections.

At our previous inspection we found that systems were not in place to ensure patients were cared for by appropriately recruited and trained staff. At this inspection we found that appropriate checks on staff had not been completed and suitable arrangements were not in place to ensure that staff received appropriate training.

At this inspection we found that patients were not protected from unsafe or unsuitable equipment because appropriate safety checks and risk assessments had not been carried out.

During a routine inspection

There was a single doctor working at the time of our inspection and we were only able to speak with three patients. They were generally pleased with the service they received at the practice. One patient described one of the doctors as, 'Brilliant'.

We saw that the provider had policies and procedures in place to help protect patients' privacy and dignity. The staff we spoke with were familiar with the procedures. None of the patients we spoke with had any concerns in this area.

We found that care and treatment was planned and delivered in a way that met patients' needs and protected their rights. Patients told us that they were able to be involved in decisions about their treatment.

The provider did not have suitable arrangements in place to ensure patients were not placed at risk of cross infection.

We were not satisfied that the provider made all the appropriate checks on staff before their full employment started to ensure that they were of good character. We were also not satisfied that the provider had suitable arrangements in place to ensure that staff received appropriate training, supervision and appraisal.
