During an inspection looking at part of the service

We undertook this follow up visit in response to concerns that we had identified during our previous inspection of the service in August 2013. Our visit was discussed and arranged with the provider in advance so that any disruption to people's care and treatment were minimised. We took an infection control specialist advisor with us to assist with our inspection.

At our previous inspection we had identified that the provider did not have effective systems in place to risk assess, prevent, detect and control the spread of healthcare associated infections. We judged at the time of our inspection that this had a moderate impact on the people who used the service and we asked the provider to take action. Following the inspection, the provider sent us an action plan which set out how they would address the issues raised.

During this inspection we spoke with four patients who used the service. None of the patients we spoke with had any concerns about the cleanliness of the practice. We also spoke with five members of clinical and administrative staff including the practice manager and a GP (who was also the provider). We found that the provider had made progress in addressing the actions required although we found areas where further action was still needed. The provider lacked a clear lead to take forward infection prevention and control issues at the practice so that infection prevention and control was given appropriate priority.

We noticed that there had been changes to the partnership at the practice since our previous inspection. We have told the provider that they must take immediate action to ensure their registration is correct and that they are operating legally.

During a routine inspection

We spoke with the provider, two practice nurses and two healthcare professionals who worked in liaison with the practice. We spoke with five patients from the practice. One patient told us, ' I've had fantastic experiences at the practice.' Another patient described their care as 'Perfect.' Patients told us they were happy with the service the practice staff provided and confirmed they felt staff were able to accommodate their needs.

We found that good communication and referral pathways existed between the practice and other healthcare professionals. Patients confirmed that their GP had supported them throughout the referral process. We saw that patients had been involved in making decisions about their care and treatment and forums existed to encourage patients to air their views.

We found that the provider needed to develop further their systems to assess the risk of and prevent, detect and control the spread of healthcare associated infection.

We found systems in place to assess and monitor complaints at the surgery. We found that the complaints process had been effective against those complaints we had reviewed.
