We looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.

At our previous assessment, the service was in breach of legal regulation in relation to lack of notifiable notifications to CQC, good governance and we rated this key question as Requires Improvement. Following this assessment, the rating has changed to Good.

Governance, management and sustainability

The service had strengthened its governance, management and oversight arrangements. Clear responsibilities, defined roles and systems of accountability were in place and were now being used effectively to manage and deliver good quality, sustainable care, treatment and support. For example, policies had been updated and reflected current practice, and there was effective oversight of recruitment records. The provider was also able to evidence improved learning from events and incidents to support service improvement and reduce the risk of recurrence. Where we had previously identified improvements were required in relation to the management of risks associated with staff immunisation records, these had been rectified. The service had not previously notified the Care Quality Commission (CQC) about notifiable events (for example, police attendance at the service) however, the provider now had a clear system to ensure staff knew and understood the process for reporting notifiable events. The provider confirmed that since the last assessment there had been no further notifiable incidents requiring notification to CQC but were able to confirm what they would do if such incidents occurred. Leaders now held regular meetings with staff where clinical concerns and emerging risks were discussed, and learning from complaints was shared. Actions were now recorded, shared with staff, and reviewed at subsequent meetings to ensure they were followed through. These meetings were now consistently documented, with actions clearly recorded and shared with the wider team.
