Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. The provider supported people to live healthier lives and, where possible, reduce their future need for care and support.

Staff identified and monitored people at increased risk of poorer health outcomes, including those living with long-term conditions, people with dementia, people with learning disabilities, carers and those approaching the end of their lives. The provider used clinical searches, audits and recall systems to support health monitoring and encourage people to attend reviews and screening appointments.

Staff supported national priorities and initiatives to improve population health, including smoking cessation, weight management and preventative care. The practice provided targeted support for people living with long-term conditions and used a personalised approach to encourage engagement with care, including making reasonable adjustments where required. Leaders demonstrated a proactive approach to population health and had developed initiatives to improve outcomes for people living with diabetes and other long-term conditions.

Monitoring and improving outcomes

The provider monitored people's care and treatment and used the information they collected to improve services. They carried out clinical audits, reviewed performance data and used searches to identify people who needed follow-up care, reviews or additional support.

The practice worked to improve uptake of screening and immunisation programmes. Childhood immunisation rates were generally high. Bowel and breast cancer screening uptake was above the national average. Cervical screening uptake was 4.6% below the national target (NHS England data from 2024). However, the practice had systems in place to improve attendance and support people to access screening. Systems were in place to identify people who were overdue for screening, with invitations and follow-up reminders sent. Where people did not attend appointments, staff reviewed individual needs, considered reasonable adjustments and offered opportunistic screening where appropriate. The practice also monitored outstanding results to support timely follow up.

From the records and evidence we reviewed, people received care and treatment in line with current guidance and standards. The practice used audits, monitoring and learning to improve outcomes for people, including those living with long-term conditions.
