Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence,choiceand control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff supported national priorities and initiatives to improve population health, including signposting for stopping smoking. We saw evidence of a 2-cycle audit to support promotion of healthier lifestyles.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service achieved 100% in relation to uptake of childhood immunisations and so performed better than the England target of 95%. The service performed below the England average target in relation to cervical screenings. In response to this the service increased the number of cervical screening appointments by expanding appointment capacity through training GPs to perform cervical screening.
