Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection at Spring Lane Surgery on 7 October 2016. The overall rating for the practice was good. This inspection was an announced focussed inspection carried out on 23 May 2017 to confirm that the practice had carried out their plan to meet the legal requirements in relation to the breaches in regulations that we identified in our previous inspection on 7 October 2016.

At this inspection we found that sufficient improvement had been achieved to update the rating for provision of safe services to good. The practice had addressed the breaches of regulation and was now compliant with all regulations. This report covers our findings in relation to those improvements.

Overall the practice is now rated as good.

Our key findings were as follows:

  • A DBS check had been completed for clinical staff.

  • Indemnity insurance was in place for clinical staff.

This is a focused inspection of Spring Lane Surgery on 07 October 2016 for one area within the key question safe. The evidence was reviewed at Spring Lane Surgery.

We found the practice to be requires improvement in providing safe services. Overall, the practice is rated as good.

The practice was previously inspected on 07 May 2016. The inspection was a comprehensive inspection under the Health and Social Care Act 2008. At that inspection, the practice was rated requires improvement for providing safe services but rated good overall.

At the inspection on 07 October 2016 we found that some improvements had been made in relation to DBS (disclosure and baring service) checks but there were still areas identified that required improvement

There were areas of practice where the provider must make improvements:

  • The provider must ensure that all recruitment checks are carried out for staff prior to employment and that all clinical staff have indemnity cover.

Spring Lane Surgery was inspected on 7 May 2015. This was a comprehensive inspection. This means we reviewed the provider in relation to the five key questions leading to a rating on each on a four point rating scale. We rated the practice as good in respect of being effective, caring, responsive and well-led and requiring improvement in relation to being safe.

Our key findings were as follows:

The practice has systems in place for reporting, recording and monitoring significant events. Significant incidents and events are used as an opportunity for learning and improving the safety of patients, staff and other visitors to the practice.

The practice has systems in place to ensure best practice is followed. This is to ensure that people’s care, treatment and support achieves good outcomes and is based on the best available evidence.

Information we received from patients reflected that practice staff interacted with them in a positive and empathetic way. They told us that they were treated with respect, always in a polite manner and as an individual.

Patients spoke positively in respect of accessing services at the practice. A system is in place for patients who require urgent appointments to be seen the same day. The practice team were very actively involved with the ‘healthier Radcliffe’ pilot project which ensures patients from six local GP practices are enabled to access GP appointments seven days a week Monday to Friday up to 8pm (and between 8am and 6pm weekends and bank holidays).

However, there were also areas of practice where the provider needs to make improvements.

Importantly, the provider must:

Improve the system of staff recruitment to ensure that patients are protected by operating effective recruitment and selection procedures that includes obtaining the required information and conducting relevant checks being carried out (and evidenced) when staff are employed or are engaged in a role where such checks are required.

In addition the provider should:

Doctors bags were managed by each individual GP. However no system was in place to regularly check the medicines contained in the doctor’s bags. Such checks should be introduced to ensure the required medicines are present and within their expiry date.
