Supporting people to live healthier lives

The practice demonstrated an outstanding and proactive approach to supporting people to live healthier lives, embedding health promotion, prevention and early intervention throughout its services. A wide range of innovative population health initiatives, including exercise programmes, swimming sessions, walking groups and a community choir, helped improve physical and mental wellbeing, reduce social isolation and encourage healthier lifestyles. The practice also maximised access to specialist weight management support, fully utilising its referral allocation and securing additional capacity to meet patient need.

 

Performance against key health indicators was consistently strong, exceeding targets for NHS App uptake, blood pressure monitoring, hypertension management and childhood immunisations. Targeted vaccination campaigns also delivered flu vaccination rates significantly above ICB averages across eligible groups. Feedback from patients and participants demonstrated improved wellbeing, increased physical activity and greater confidence in managing their own health.

 

The practice extended its preventative approach beyond its registered population through collaborative work with local schools, delivering mental health workshops, careers support and access to a Health and Wellbeing Coach. This resulted in sustained engagement from young people, improved confidence and emotional wellbeing, and earlier intervention before needs escalated.

 

The service showed a highly responsive approach to supporting vulnerable groups. Annual health checks for people with a learning disability increased from 43% to 100%, supported by proactive care home visits and strong continuity of care. Dedicated continuity arrangements for people living with dementia were valued by families and improved care experiences. The practice also ensured safe transitions between private and NHS care, reducing risks of treatment disruption.

 

Strong partnership working with domestic abuse services enhanced the identification, safeguarding and support of people at risk. The practice demonstrated effective advocacy for vulnerable patients, used learning from safeguarding cases to strengthen pathways and worked proactively to address barriers within the wider system.

 

Overall, the practice consistently went above and beyond to improve population health, reduce inequalities and promote independence. Its innovative, community-focused and preventative approach delivered measurable improvements in outcomes and demonstrated outstanding practice.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

 

The service met national targets for the uptake of childhood immunisations. The service performed better than the England average in relation to several cancer screening programmes, including screening for breast cancer (87% compared with national average of 70%), and bowel cancer (81% compared with national average of 72%).

 

The provider demonstrated strong clinical leadership in the prevention and early detection of long-term conditions. For example, the implementation of a lipid optimisation pathway significantly reduced people’s risk of cardiovascular complications, while the introduction of innovative diagnostic equipment enabled early detection of atrial fibrillation (irregular heartbeat). These initiatives ensured timely diagnosis and access to effective treatment, improving outcomes and reducing the risk of avoidable harm.
