Dalefield Surgery, Avondale Health Centre, Avondale Street, Bolton, BL1 4JP provides commissioned services under the General Medical Services (GMS) contract within the Bolton Commissioning Group area.

The practice is responsible for providing treatment to approximately 6300 registered patients and offers services that include meningitis provision, the childhood vaccination and immunisation scheme, extended hours access, facilitating timely diagnosis and support for people with dementia, influenza and pneumococcal immunisations, patient participation, rotavirus and shingles immunisation, unplanned admissions and contraceptives.

The practice consists of two GP partners and one salaried GP, one advanced nurse practitioner one practice nurse and one health care assistant . The practice is supported by a practice manager and an administration team, receptionists and apprentices.

Information taken from Public Health England placed the area in which the practice is located as two on the deprivation scale of one to ten. (The lower the number the higher the deprivation). In general, people living in more deprived areas tend to have greater need for health services.

Data shows that the age profile of the practice population is broadly in line with the CCG and national averages.

Dalefield Surgery is a training practice where one of the doctors is a trainer to doctors who are undergoing their post graduate training.

We carried out an announced comprehensive inspection at Dalefield Surgery on 12 June 2018 as part of our inspection programme.

At this inspection we found:

• The practice had clear systems to manage risk so that safety incidents were less likely to happen. When incidents did happen, the practice learned from them and improved their processes.
• The practice routinely reviewed the effectiveness and appropriateness of the care it provided. It ensured that care and treatment was delivered according to evidence- based guidelines.
• Staff involved and treated patients with compassion, kindness, dignity and respect.
• Patients found the appointment system easy to use and reported that they were able to access care when they needed it.
• There was a strong focus on continuous learning and improvement at all levels of the organisation.
• The practice manager had been in post since February 2018.
• The practice had changed its clinical system six days prior to the inspection.
