During an inspection in response to concerns

We carried out this inspection because of concerns found during our previous visit on 17 July 2013. We also received information of concern from Birmingham, Black Country and Solihull NHS England Area Team (AT) and Solihull Clinical Commissioning Group (CCG). Their role concerns ensuring quality of care for patients. During our inspection, we spoke with two doctors and five members of staff. We later spoke with six patients by telephone.

Patients had not received appropriate care and treatment. They had concerns about the practice, difficulty making appointments and excessive waiting times to see doctors. One patient said: 'I had to wait an hour and a half yesterday.' Three patients had concerns about cleanliness. One told us: 'It could do with a spruce up.' Three patients told us their long term medication had not been reviewed. We found concerns with infection control and medicines management. This meant patients were at risk of not receiving appropriate care and treatment.

Since this inspection the GP partnership registered to provide services at Kingshurst Medical Practice has voluntarily cancelled their registration with the Care Quality Commission. An alternative provider is now registered to provide services there. This action has been taken by the practice, CQC, AT and CCG to address serious concerns and ensure that patients did not lose access to a GP service. This report does not comment on services provided by the alternative provider.

During an inspection in response to concerns

We carried out this inspection because we had received information of concern from Solihull Clinical Commissioning Group (CCG). This information related to the management and staffing of the practice and the impact of this on the service patients were experiencing. Their role concerns ensuring quality of care for patients.

Our inspection was unannounced. We spoke with five patients and six members of staff. We later spoke with five patients by telephone and seven patients who had made complaints.

Patients had not received appropriate care and treatment. Patients said they lacked confidence in one of the doctors, had difficulties in making appointments and experienced excessive waiting times to see doctors. Comments from patients included: 'When Doctor A moans to me it goes on for a long time and that delays patients from being seen. I now avoid seeing Doctor A'. 'I have waited up to two hours to see the doctor but have waited longer'. 'Lots of patients have moved to other practices'.

Patients needs were not being met due to staffing shoratges. Management of staff was ineffective and staffs appraisals were overdue. No systems were in place to ensure that nursing staff maintained their professional registration. This meant that nurses had been working illegally.

The provider failed to adequately monitor the quality of service provision. Complaints were not responded to appropriately. We were not assured that patients received good standards of care.

Since this inspection was undertaken the GP partnership registered to provide services at Kingshurst Medical Practice has voluntarily cancelled their registration with the Care Quality Commission.

An alternative provider is now registered to provide services for patients from Kingshurst Medical Practice. This action has been taken by the practice, CQC, Birmingham, Black Country and Solihull NHS England Area Team and Solihull CCG to address serious concerns and to ensure that patients did not lose access to a GP service. This report does not comment on the services provided by the alternative provider.
