Well-led
We looked for evidence service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

The service had a shared vision, strategy and culture. This was based on transparency, equity, equality, human rights, diversity, inclusion and engagement. Staff and leaders understood the challenges and the needs of people and their communities.
The service had a clear vision, values and strategy that focussed on patient-centred care, safety, equality, staff wellbeing and continuous improvement. Staff were aware of these values and passionate about providing the best care for patients. During our assessment, staff spoke positively about leadership and described supportive working relationships. Staff described how leaders provided practical and emotional support. Comments included, “GP support all the way and never felt can never ask”, “Very privileged to work here and it’s a good team to work”, “Encouraged to come and work in a happier environment”.
Leaders understood the needs of the local population and worked with the PCN and PPG to support service development and address local health inequalities. Patient’s feedback recognised improvements at the practice. For example, a patient told us, “Following our feedback, the seating arrangements in the reception has changed”.
The service had a diverse workforce that reflected a range of backgrounds and experiences.

Governance, management and sustainability
The service had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate.
The provider had governance processes appropriate for the service, which included core policies, standard operating procedures and a business plan. Although staff had protected time allocated for mandatory training, records were not consistently up to date. The practice manager had recognised this issue and had developed an action plan to ensure outstanding training was completed.
All staff we spoke with understood their individual roles and responsibilities and described leaders as visible, supportive and approachable.
Leaders proactively supported staff and regularly met with them to complete appraisals and discussed emerging risks and potential improvements to the service. Staff were aware of the importance of patient confidentiality and information security.
The results from the GP National Survey had been consistently positive compared to local and national averages.
