Clinical searches and alerts were used to identify people at risk of poorer outcomes on the practice IT system, including those with long-term conditions, vulnerable patients and people approaching the end of life.

People had access to health checks, screening, smoking cessation, weight management, specialist diabetes services, social prescribing and community wellbeing support.

Weekly quality outcomes and long-term condition meetings were held, and clinical decision-support software had been introduced to prompt opportunistic action to monitor and ensure people’s health and care needs were being met.

National cancer screening and childhood immunisation uptake data was reviewed and targeted improvement measures were used, including to identify barriers to approach care such as additional staff time for childhood immunisations and a delegated named staff lead, recall lists, and a cervical screening failsafe reports and a 3-stage recall process. Cervical screening could also be accessed through early morning, evening and weekend Primary Care Network appointments.
