Safe environments

Score: 3

The service detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.
The service had a business continuity plan which was regularly reviewed and outlined how the service should continue to operate in the event of a disruption. The service had contracts to ensure the premises was maintained. Staff and leaders completed health and safety risk assessments and undertook audits to ensure they had identified and addressed all risks. At the time of the inspection, scaffolding was in place to facilitate roof cleaning works. The premises were warm, clean and appropriately maintained. Access to the building and consultation rooms was suitable for patients' using wheelchairs, mobility aids and prams.
The service had processes in place to manage fire safety risks; however, these were not always effective in identifying and mitigating all risks. For example, whilst a fire risk assessment had been completed, it did not fully consider the arrangements required to safely evacuate patients and visitors with reduced mobility from the first floor in the event of an emergency. The provider was reviewing the availability of appropriate evacuation equipment and staff training to support safe evacuation.

Leaders ensured staff were up to date with their training which the service had deemed mandatory and operated within their agreed areas of competence. The service followed safe recruitment procedures when employing staff, which were in line with national legislation. This included identity checks, review of qualifications, obtaining of professional references and a criminal records check. The service employed a range of clinical and non-clinical roles and employed GP Registrar’s who were appropriately supported through supervision by appointed GPs.

Staff had completed relevant training in infection prevention and control. The service had cleaning schedules available, which outlined how staff should clean the building and its equipment. The service demonstrated how these were monitored for completion to maintain oversight of cleaning arrangements.
During our onsite visit, the service's premises and a sample of equipment reviewed was noted to be visibly clean however, carpeted areas were present within GP consultation rooms. The IPC lead was aware of this and had implemented plans to address the issue and ensure the practice achieved full compliance with infection prevention and control standards.
