Shared direction and culture

The provider had a shared vision, strategy and culture which staff understood and supported. The practice had developed an in-house vision, which was embedded within policies, displayed throughout the practice and regularly reinforced with staff.

Leaders maintained oversight of the future development of the practice through a business development plan covering 2025 to 2028. The plan included succession planning, service development priorities and proposals to re-establish the Patient Participation Group (PPG), supported by defined timescales and associated costs. Leaders monitored progress against the plan to support the delivery of strategic objectives.

Staff and leaders understood the needs of people and the local community. They promoted a positive and compassionate culture, supported by an open-door approach from leaders. Staff wellbeing surveys were undertaken, and leaders encouraged staff to provide feedback. When communication was identified as an area for improvement due to the high proportion of part-time staff, leaders responded by increasing the frequency of meetings, sharing meeting minutes and introducing a weekly staff newsletter.

Leaders promoted equality and diversity within the workforce. Relevant policies and training were in place to support an inclusive working environment.

Governance, management and sustainability

The provider had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate.

Staff understood their roles and responsibilities, including those with designated lead responsibilities, such as infection prevention and control. The provider had a clear leadership structure and leader’s maintained oversight through regular leadership, safeguarding, palliative care, clinical and whole-practice meetings. A weekly newsletter supported communication and engagement across the workforce.

Governance processes for the quality statements we assessed were appropriate to the service. Policies and procedures were managed electronically, overseen by the Practice Manager and Deputy Practice Manager, and subject to regular review. Leaders used audits, complaints and significant events to monitor and improve quality. For example, a complaint relating to the communication of sensitive information led the practice to strengthen its protocol for communicating with patients under the age of 16, with greater consideration given to privacy, confidentiality and individual circumstances.

The practice also used audits to identify opportunities for improvement and strengthen governance processes. A safeguarding audit reviewed staff awareness of safeguarding arrangements and identified opportunities to improve access to safeguarding information. This audit did not assess compliance with safeguarding training requirements but focused on how staff accessed safeguarding information. As a result, the practice updated its safeguarding policy to include local safeguarding contacts and introduced a safeguarding poster detailing safeguarding leads, escalation processes and key contact information to support staff awareness and reporting of concerns.

Systems were in place to manage safety alerts and statutory notifications, ensuring information was shared with external organisations as required.
