Well-led
We looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

The service had a shared vision, strategy and culture that reflected transparency, equality, diversity and inclusion. Staff and leaders understood the needs of their population and the challenges within the local community.
The service had a clear set of vision and values, which were displayed in a mission statement across both sites. Staff we spoke with were aware of these values and described a commitment to delivering patient-centred care, with a focus on quality, safety, compassion and respect.
Staff and leaders demonstrated a positive and supportive learning culture. Staff told us they felt encouraged to develop their skills and were supported by leaders within the service. They described the culture as open and inclusive, and said they felt valued in their roles.
Leaders had oversight of the service and monitored progress against priorities, using feedback and performance information to support continuous improvement and delivery of care that met the needs of patients.

Governance, management and sustainability
The service had clear responsibilities, roles, systems of accountability and good governance. These were used to manage and deliver safe and sustainable care.
Staff we spoke with demonstrated a clear understanding of their roles and responsibilities, including those with lead roles such as infection prevention and control. There were arrangements in place for the effective management and oversight of staffing, policies and procedures. Staff were able to access policies and procedures when required, and these supported the delivery of care. Leaders were visible and approachable, promoting a culture of openness and accountability.
There was a structured meeting process in place across the service. Meeting minutes were recorded and made available to staff, including those unable to attend, through the shared system. This supported communication and ensured staff remained informed of key updates and decisions.
The service had effective arrangements for the management of data and records. Staff understood the importance of maintaining confidentiality and information governance standards, and there was a clear focus on maintaining high standards of data quality and security.
Succession planning had been considered to support the sustainability of the service. Staff we spoke with provided positive feedback about the culture within the service, describing feeling supported, valued and respected.
Governance systems had identified that environmental risk assessments at the Garforth site required review. Leaders had recognised the legionella risk assessment was out of date and had implemented interim controls while actions were taken. They had also arranged for external review of the fire risk assessment, demonstrating responsive oversight, although systems had not ensured all assessments remained current.
A Patient Participation Group (PPG) was established, with ongoing recruitment of members. Feedback from PPG representatives was positive, and they described the service as inclusive and well-led, with approachable leadership and a supportive team environment.
