Background to this inspection

Updated 30 May 2018

Lime Tree and Sinnott Healthcare is located in a residential area in east London based in a converted house. There were 6463 patients registered with the practice 25% of whom did not have English as a first language. The practice scored two on the index of multiple deprivation score where a score of one represents the most derived and 10 is the least deprived.

The practice has one female GP partner, two female salaried GPs and two male sessional GPs, who complete a total of 32 sessions per week; there was also a female practice nurse who completes eight sessions per week. The practice has one practice manager, an assistant practice manager and six administration and reception staff members. The practice worked closely with a neighbouring practice who it shared staff members with.

The practice operates under a Personal Medical Services (PMS) contract (a locally agreed alternative to the standard GMS contract issued when services are agreed locally with a practice which may include additional services beyond the standard contract).

The practice is open Monday to Friday between 8am and 6:30pm where extended hours are on a Monday evening until 8pm and Tuesday and Wednesday evening until 7:30pm. Telephones are answered from 8am and appointments times are as follows:

Monday 8am to 12:45pm and 2pm to 7:50pm.

Tuesday 8am to 12:45pm and 2pm to 7:15pm.

Wednesday 8am to 12:45pm and 2pm to 7:15pm.

Thursday 8am to 12:45pm and 2pm to 6:15pm.

Friday 8am to 12:45pm and 2pm to 5:20pm.

The locally agreed out of hours provider covers calls made to the practice when the practice is closed and the practice is a part of the local HUB which provides GP and nurse appointments to patients on weekday evenings and on weekends when the practice is closed.

Lime Tree and Sinnott Healthcare operates regulated activities from one location and is registered with the Care Quality Commission to provide treatment of disease, disorder or injury, diagnostic and screening procedures and maternity and midwifery services.

We carried out an announced comprehensive inspection at Lime Tree and Sinnott Healthcare on 12 March 2018 as part of our inspection programme.

At this inspection we found:

  • The practice had clear systems to manage risk so that safety incidents were less likely to happen. When incidents did happen, the practice learned from them and improved their processes.
  • The practice routinely reviewed the effectiveness and appropriateness of the care it provided. It ensured that care and treatment was delivered according to evidence- based guidelines.
  • There was evidence of quality improvement and the practice made good use of clinical audits.
  • Clinical guidelines and patient safety alerts were discussed in clinical meetings where learning was shared.
  • Staff involved and treated patients with compassion, kindness, dignity and respect.
  • Patients found the appointment system easy to use and reported that they were able to access care when they needed it.
  • The practice had clear systems for acting on responding to and learning from complaints.
  • There was a strong focus on continuous learning and improvement at all levels of the organisation, all staff had completed mandatory training and were given the minimum of one afternoon a month protected time to keep up to date.

The areas where the provider should make improvements are:

  • Ensure systems are in place to label all clinical waste bags.
  • Continue to review the system for reviewing patient pathology results.
